Self Employment Program Activity Report

Self Employment Program clients can now complete their Monthly Activity Reports online. Please note that all Self Employment forms are now in Adobe Acrobat PDF format. There are 3 ways to submit your report.

1. Online*. Complete the PDF form and press the submit button.
2. Offline**. Complete the PDF form, print it, then fax mail or deliver it.
3. Offline**. Print the form and fill out it at your convenience, then fax mail or deliver it.
